Requirements

Veterans (separated from active duty):

  • DD-214 Member Copy 4 (Certificate of Release or Discharge from Active Duty)
  • Must show character of service (honorable preferred)

Active Duty Service Members:

  • Statement of Service signed by your unit's adjutant, personnel officer, or commanding officer
  • Must include: full name, SSN, date of birth, entry date, duration of lost time, name and title of signatory

National Guard / Reserves (never federally activated):

  • NGB Form 22 (Report of Separation and Record of Service) for National Guard
  • NGB Form 23 (Retirement Points Accounting) — showing 6+ qualifying years
  • Copy of your discharge certificate or release document

Guard/Reserve (federally activated with Title 10 orders):

  • DD-214 from the period of activation
  • Activation orders if DD-214 is not available

Surviving Spouses:

  • Veteran's DD-214
  • Marriage certificate
  • Veteran's death certificate
  • VA Form 26-1817 (Request for COE for Surviving Spouse)
  • DIC award letter (if receiving DIC)
